Medical Case Management

OHIOcares purpose is to enhance the “safety net” of behavioral health services available for military personnel and their families including the services available through the Department of Veterans Affairs and Vet Centers. Providing command with the tools they need to guide service members in need is critical to accomplishing this. Guard members may be identified with medical needs during health assessments, self-referrals or as a result of command referrals. A medical provider reviews the condition and the guard member may be deemed medically non-available by issuing a temporary profile. A medical case manager completes an Individual Medical Action Plan (IMAP), detailing the medical issue and the next required guard member action. Upon completion of the IMAP, the medical information is pushed back up through the chain of command by the medical readiness team. The case manager compiles the case packet for medical review. The medical provider reviews the case and may; return the guard member to duty, send the case to the board, or send the IMAP back for further tasks and extend the profile.
